Follicular Unit Extraction (FUE)

Follicular Unit Extraction (FUE) is a method of single follicular unit harvesting directly from the healthy donor area on the scalp (usually at the back and sides of the head), transplanting into the balding area as individual follicular units (1-3 hairs) for natural appearance. These are two main advantages of this technique: the lack of a linear scar, such as those originated with strip harvests; and a rapid recovery time, with less postoperative pain and discomfort.

This procedure is carried on under local anesthesia or intravenous anesthetic sedation by the doctor only. This method uses a specialized punch device, ranging in diameter from 0.8-1.2 mm, making a tiny circular incision in the skin around a follicular unit, separating it from the surrounding tissue and avoiding the damage to individual hair follicles. The single follicular unit is then extracted directly from the scalp, with tiny white scars buried in the hair at punched area after one week of the procedure. The follicular units are then inserted into the recipient areas where they will produce into healthy hair follicles.

The recipient sites making and the insertion of follicular unit grafts are necessarily the same in both FUT and FUE procedures. The variety remains in the presence of the donor area and in the amount and aspect of grafts acquired with each technique. The other primary season for FUE chosen by patients is the request to avoid a linear scar.

Follicular Unit Transplantation (FUT)

Follicular Unit Transplantation (FUT) is an original hair transplant procedure which is the harvest of donor hair from the permanent zone at the back and sides of the scalp in a single, thin strip and then dissected into in individual follicular units under microscopes. The principle of this procedure is that the hair is transplanted into the balding area as individual follicular unit (1-3 hairs) for natural appearance. This procedure is performed under local anesthesia or intravenous anesthetic sedation by the doctor only.

Donor harvest planning should be done keeping future hair recession in mind with minimization of follicular transection. We perform the wound closure using trichophytic technique with absorbable sutures (that no need to remove) for tension free, to give best possible scar.

The period of sessions will depend upon how many follicular unit grafts are transplanted. Small sessions of less than 1,000 grafts can be finished within 6 hours. The larger sessions, of 2,500 grafts or more, usually take at least 12 hours.

Platelet Rich Plasma (PRP) for Alopecia Treatment

What is PRP?

PRP means Platelet Rich Plasma.

PRP is used for therapeutic applications, other than hematological treatment since the end of the 20th century, after the discovery that it contains growth factors. Used to stimulate tissue healing and regeneration. Platelets play crucial role in hemostasis and blood clot formation. They contain granules full of clothing mediators and growth factors (GF). Plasma is the liquid part of the blood which contains cell nutrients, vitamins, hormones, electrolytes and growth factors (IGF, HGF). It also contains proteins (albumins, globulins, clothing factors, and fibrinogen).

How many types of PRP? and What are the differences?

The technology used for PRP preparation affects its composition not only platelet concentration but also the level of contamination with red and white blood cells.

Alopecia Treatment Pattern

Point by point injection technique

PRP Treatments combination with Hair Transplant

Pre-treatment: 1 to 2 tubes of PRP mesotherapy injections of graft donor and recipient sites to be performed 1-2 weeks prior to grafting.
Hair graft day: collect follicular unit grafts and place in PRP for follicular nourishment until re-implantation
Directly post grafting, PRP could be applied topically on donor and recipient sites.
Post graft treatment: 1 to 2 tubes of PRP mesotherapy injections of graft donor and recipient sites to be performed 3-4 weeks post-grafting.
Follow up treatment: Booster at 6 months
PRP stimulates and supports hair growth in three main steps:

Promoting vascularity to the hair root thereby increasing viability to the follicle before, during and after hair transplantation
Promoting tissue repair and regeneration post transplantation
Stimulating dormant hair follicles and growing new hair

Medical Treatment

We use medical treatment as a supplement to surgical procedures. Minoxidil and Finasteride are the two well-known medicines for the treatment of common hair loss (androgenetic alopecia). Both these medicines act at different levels; while finasteride is androgen modulator, minoxidil is primarily a hair growth stimulator. For this reason, it is best to use them simultaneously when hair loss is progressive or severe. We also have other option in medical treatment category such as Anti hair loss serum, Low-level laser therapy (LLLT), and Aroma massage shampoo.

Finasteride

Finasteride is the single most effective medication proven to treat genetic pattern hair loss. Finasteride works by acting as a potent and specific, competitive inhibition of 5 alpha-reductase, specifically the type II isoenzyme. By inhibiting type II, 5alpha-reductase it causes a decrease in serum dihydrotestosterone (DHT) levels by about 65-70 percent. This reduces DHT induced perifollicular inflammation and partial reversal of scalp hair miniaturization as evidenced by histopathologic evaluation of scalp biopsies.

The clinical use of finasteride for the treatment of androgenetic alopecia (AGA) in men dosage is 1mg daily. The clinical studies at 1 year, showed 12 percent increase in hair count and 48 percent increase in hair growth for finasteride group. Those patients who stopped finasteride lost the hair gain.

The finasteride-related sexual dysfunction is the main side effects which can occur varying from 1.1 to 18.5%. These effects are generally reversible after stopping the medicine. The recent study in this year (2019) shows that the use of finasteride does not result in sexual dysfunction in men with AGA. This reflect recent research which suggests that men with alopecia may be inherently more susceptible to sexual dysfunction regardless of treatment.

Minoxidil

Minoxidil is an arteriolar dilator introduced in clinical practice in the 1970s for the treatment of hypertension. Minoxidil acts on three levels for hair growth effects.

1 Induces cell growth factors in dermal papilla cells, so that prolonging anagen phase and contributing to increased hair shaft diameter.

2 Inhibits of transforming growth factor-beta (TGF-B) induced apoptosis of hair matrix cells, therefore prolonging that anagen phase.

3 Dilates hair follicle arterioles and increases blood flow in dermal papilla.

Primary indication of minoxidil is in male pattern baldness. Minoxidil seems to reduce the rate of hair follicle miniaturization, increase in thickness of hair and prolongation of anagen phase. All these effects combine to increase total hair mass and consequently coverage. Currently, 2-5 percent minoxidil as lotion used twice daily is considered first time treatment for male pattern hair loss but 56 percent solution is usually preferred over 2 percent solution.

Some side effect can happen after minoxidil usage such as hypertrichosis with hair in undesirable areas like over arms and face due to systemic absorption of the topical drug. After discontinuation of minoxidil,about 3 months the hairs usually disappear but may rarely require LASER depilation. Local skin reactions like pruritic and allergic contact dermatitis may also occur but our formula develop for less skin reactions to all types of scalp skin.
